Insurance & Payment Information

There are many factors that play into how much your insurance company will pay for treatment costs. This document provides a general overview. Because each person’s insurance plan and situation vary, we strongly recommend you speak with one of our Admission Counselor’s to obtain information specific to you. Please call (833) 792-1489, and ask for someone in Admissions. We pride ourselves in providing concierge service -if we cannot help you, we will refer you to someone who can.

When you call, we will need the following information to check your insurance benefits:

• Name of insurance company
• Mental Health/Substance abuse or Insurance provider services phone number
• Member ID and group number
• Name of policyholder
• Policy Holder birthdate
• Name of client
• Client birthdate
• Client address (the one insurance has on file)
• Client phone number (the one insurance has on file)
• Name of employer the coverage is through

We are a preferred provider with a majority of private insurances either through employers or self-funded. Our contractual relationship with in-network providers allows us to collect any unmet in-network deductible, copayment or out-of-pocket maximum for days covered by insurance.

We can work with any other insurance plan that offers “out-of-network” coverage on a case by case basis.

You can expect minimal residential coverage thru Magellan and United Health Care/United Behavioral Health/Optum out of San Francisco. Typically Azure has seen them authorize between 8-15 days total residential services. Magellan does not cover detox.
